How can health clinics make visits by patients more efficient? Startup Care Directions believes it has the answer.

Better digital tools.

"Care Directions is a corporation established in June 2017 to commercialize applications to generate individualized care plans (COMPASS-CP) and related applications to transform patient engagement and empowerment to improve their health, independence, and recovery," says CEO Pamela Duncan.

"Researchers at Wake Forest Baptist Medical Center have developed COMPASS-CP, a patient-centered electronic application that captures the social and functional determinants of an individual’s health at the point of care. Designed to be administered by a clinician in a clinical or home setting, the tool also assesses caregiver abilities and resources critical for patients during the post-stroke care period. Housed as a web-based or iPad application, the functional assessment is simple to administer, yet provides a comprehensive overview of potential barriers that can impair a patient’s ability to manage their personal health and recovery."

The startup already has received $300,000 in funding from the Wake Forest Innovations Catalyst Fund.

The startup, which is based at Seven lakes, N.C., will be demonstrating its technology at the CED Tech Venture Conference on Sept. 19-20 in Raleigh.
